Case Logs

The ToxIC Registry contains no protected health information (PHI). The only patient specific information on the Registry is a unique code field assigned by each ToxIC site. These unique codes are managed at the individual participating site as part of a site case log.

Therefore, only the individual site may access the identity of the cases for data quality activities or appropriately IRB-approved research studies via follow back to this log.

The format of ToxIC case identification numbers are defined by the individual site. However, they should not be identical to the medical record number used at any individual clinical facility.  In addition, the unique code must be unique meaning no code may be used more than once.  In general, most ToxIC sites use a combination of alpha numeric characters ranging from 8 to 16 digits.
