TABLE 1:  Common Sources of Error in Transcription and Their Prevention



Illegible orders - Block printing or Automated Physician Order Entry
Decimal Dosing Errors

- Do not use trailing zeros (20 not 20.0)
- Do not abbreviate units (20 units, not 20u)
- Always use leading zeros (0.2 not .2)

Look-alike medications - Emphasize differences (doPAmine, doBUTamine)
- Include indication (Celebrex [arthritis], Cerebyx [seizure])
Verbal orders - Separate numbers, syllables ('one six', not '16' [sounds like 60])
- Require read-back
